The only game the 3-Up Moon ever appeared in was Super Mario World and its several remakes and re-releases. If Mario finds this item it gives him three extra lives. This is one of the rarest items in the game. 1-Up Mushrooms in comparison only rewarded Mario with 1 extra life and are much easier to find.
